if the alert has no defined recipients, then it is sent to all devices that have downloaded the app and accepted the prompt asking to receive push notifications

if any groups are added to the alert, then it is sent only to those groups. The intended recipients are required to have signed in at least once to be able to receive an alert sent to a group
